Workers Compensation Office Policy
When you send an injured employee to our office for treatment, if you elect to proceed through workers' compensation insurance for this work related injury we require the following:
- Confirmation by the employer that the injury did occur at work as described
- Employer's workers' compensation insurance carrier
- The case number for the injured employee which is obtained by the employer from the workers' compensation carrier.
We will not be able to bill the workers' compensation carrier without this information.
Without this information we will require a "hold payment" for treatment of the injured employee for services rendered until we receive this required information. As soon as we receive this information we will release the hold payment and file the claim through the carrier. If we do not receive this information within 2 weeks the "hold payment" will be transacted and considered payment for the visit.
A "hold payment" may be made by company check or credit card .
You can call our office and ask for Heather if you have any questions. The form for providing us with this required information can be obtained here and faxed to us “Attention Heather” (1-631-587-1364). The employee may also be able to bring a completed form to the office at the time of treatment.
Established company pay accounts will be unaffected by this new policy.
Companies that desire to pay for the medical care will be required to pay for the treatment at the time of the visit until we can establish a company pay account.
IMPORTANT
If your employee presents to our office for treatment of a work related injury which requires immediate medical care and we do not have the required information, no hold payment is given or the company cannot pay for the visit we will provide first aid to the injured employee and send the employee to the nearest emergency room or other facility as determined by the employer.
